According to the analysis by Saunders (2014) and contemporary nursing scholarship, the progress of Advanced Practice Registered Nursing (APRN) has been historically hindered by systemic, legislative, and professional obstacles.

Below are the three primary barriers identified and the strategic solutions proposed to overcome them.

Variation in State Scope of Practice (SOP) Regulations

Perhaps the most significant barrier is the "patchwork" of state laws. While some states grant Full Practice Authority (FPA), others require "Reduced" or "Restricted" practice, where an APRN must have a formal collaborative agreement or direct supervision by a physician to diagnose or prescribe.

The Problem: This inconsistency creates "healthcare deserts," as APRNs are often unable to practice to the full extent of their education in the areas that need them most. It also complicates the portability of licenses across state lines.
