Start work on your evaluation plan with a diagram and the rest will fall into place. If you can't draw a diagram that makes explicit what the intervention involves and the underpinning assumptions about the interventions change mechanism (the logic model/theory of change), then it is difficult to evaluate. This is because you probably don't adequately understand what you are evaluating and/or you won't be able to identify what and when data needs to be collected to assist evaluation.
