A Feasibility Study for Developing a Multidisciplinary Post Discharge Cardiac Care Telehealth Program.
A literature review chapter within a 15k word feasibility study
that aims to assess the viability of a telehealth program for cardiovascular medicine (CVM) and cardiothoracic surgery (CTS) patients that will reduce the
number of readmissions to the hospital and improve the patient’s post-discharge experience.
