nursingcare plan

nursingcare plan Order Description   Assessment 2: Written assessment - scenario based nursing care plan Number of words: 1000 The written assessment task is related to Mr Peter Jones, your Inquiry Based Learning (IBL) case study patient in week three tutorial. The first part of this assessment task should include brief evidence-based justification of the three problems that have been used in the care plan. The second part is the care plan that must include a patient problem statement, goal, interventions, rationale, and evaluation. Further details: As a nurse caring for Mr Jones on the third post-operative day, using clinical reasoning skills, what would you identify as the top three priority problems for Mr Jones after a laparotomy and a newly created colostomy? You also need to consider the fact that Mr and Mrs Jones also need to cope with a cancer diagnosis; therefore, psychosocial implications of this new diagnosis should also be taken into consideration. For the three high priority problems chosen from Mr Jones’s case scenario, you will need to produce a care plan with a problem statement, goal, interventions, rationale, and evaluation. The tutorial discussion in week 3 will enable you to do a detailed case analysis on Mr Jones that may help identify some of the priority areas in the post-operative phase of his care. In addition, the week 1 lecture will help you understand the elements of care plan writing. Please note that the care plan submitted for this assignment needs to be an individual piece of work. This is not an essay and therefore does not require an introduction and conclusion. Headings should be used in the first section where you justify the three chosen problems. Answers should be written in a scholarly style. The Nursing Care Plan must be referenced and related specifically to Mr Jones. You should include as many interventions as you determine are necessary to fully manage the chosen problems. A simple problem may have fewer nursing interventions than a complex problem. Each of the intervention provided should be accompanied with a rationale and an evaluation. The nursing care plan can be set out in a table format. A proforma table has been provided on VU Collaborate for you to follow, however its’ use is not compulsory. All written assessments must align to scholarly standards. Academic standards require: • APA 6th edition for in-text citations and reference list; • adherence to word count; • a 12 size font in either Arial, Times New Roman or Calibri; and • a 1.5 spacing. 1) How do I gain more information about Mr Jones’ surgery and post-operative nursing care? As you work on your care plan assignment it is important that you read further on the surgery that Mr Jones underwent (laparotomy and relating procedures for a recto-sigmoid tumour, colostomy) and post-operative nursing care. Also I would recommend you to refer a few medical-surgical textbooks from the library and look at some care plan examples. This would give you a general idea on establishing nursing care priorities which you can then relate to Mr Jones and his current needs. 2) Am I allowed to choose a potential (at risk) problem or does it all have to be actual problems? You can choose actual or potential problems as long as you are able to justify in the first section as to why you think the problems you have chosen are of a high priority for Mr Jones in the post-operative phase. Also make sure you use problem statements from NANDA-I’s approved list which is provided on VUCollaborate. 3) Is there a minimum word count for the justification of problems? The stipulated word count (1000 words) for this assignment includes the entire paper except for the title page and references. The first section (that is justification of problems) is expected to be short paragraphs with literature evidence supporting the significance of the chosen problem in Mr Jones’ context. There is no fixed word count for the first section, but remember you need to also develop a detailed care plan so use your words wisely. 4) Is the problem statement same as nursing diagnosis? Identification of patient‘s problems is the first step which is then followed by choosing nursing diagnosis labels for the identified problems. So for example if stress relating to the current surgery is identified as a problem, the nursing diagnosis label could be ‘ineffective coping,’ or perhaps ‘fear & anxiety’ which is suitable label obtained from the approved list of NANDA-I 5) How many goals do I need for a nursing diagnosis? You only need one goal statement for the problem you have chosen. For example if ineffective breathing pattern….is chosen as the diagnosis a goal statement would be “Improvement in breathing pattern.” Mr Peter Jones is a 62-year-old man who lives in a Western suburb of Melbourne. He retired a few years ago after many years of service as a truck driver. He weighs 84kgs, leads a sedentary lifestyle and spends most of his day watching television. He is a smoker and drinks about four stubbies a day. He also has hypertension for the past eight years. For the last couple of months, Mr Jones has been noticing blood in his stool but ignored it and did not care to mention to Mrs Jones. In the last week, he has also been experiencing abdominal pain that comes and goes in waves and he is very constipated. Since this morning, he has been experiencing increased bloating and discomfort in his abdomen, which then progressed as severe abdominal pain, not relieved by any remedies offered by Mrs Jones. Soon he was taken to the emergency department of the closest hospital. Initial abdominal x-ray and a CT scan revealed a tumour obstruction in the recto sigmoid region of the colon and multiple lesions in the liver.Mr Jones was taken in for an emergency laparotomy for the resection of the recto-sigmoid tumour. Post operatively he has a stoma and a large dressing covering his laparotomy wound Suggested structure for this task – No introduction or conclusion is required.
