This paper is a spin off of my last paper Transitional research re: Lack of communication between nurse and patient causing increase in ER return admissions, and patient dissatisfaction (resulting in poor HCAHPS scoring). More emphasis needs to be put on Achieving an exceptional patient and family experience to increase patient overall health, education and understanding of disease process and plan of care as well as outcomes. Please leave section B. blank with just a header for the insertion of the PFCC tool assessment. For # B1. I will be focusing on a County hospital in Moreno Valley, Calif, which primarily serves the underinsured, primarily hispanic, safety net population, I will upload the self assessment tool and you may choose which strengths/weakness you would like to focus on for section B2. and C.
