Next, list the purpose of the Consent Form and what it is being used for. In this case, testing and assessment to aid in formulation of a treatment plan.
Include risks and benefits of tests and assessments in a short paragraph or a short list.
Explain what will be done with the results of the testing and assessment: how they will be stored/kept, shared with other practitioners, and utilized in the client's treatment.
