Screen Design - Human Computer Interaction Usability

NSU is hosting a scavenger hunt for teams of between 3 to 5 people. All team members must wear matching shirts with FIU’s logo and a team name. Each team leader must place only one order for all of the team’s shirts.
The information that must be collected from the user on the order screen includes:

1. Customer name
2. Customer address, city, state, zip
3. Customer phone number
4. Customer email address
5. Customer payment method (credit card, Paypal, or check)
6. Shirt type (short sleeve, long sleeve, or sleeveless muscle shirt)
7. Shirt color (white, blue, or gold)
8. Shirt size (small, medium, large, or extra large)
9. Number of shirts in each size
10. Team name
11. Delivery option (pick up or ship to customer)
12. Express shipping option
