The treatment plan allows you to take what was learned during the intake session(s) and make a plan to meet the
client’s goals. Continue using the same client you selected for the intake assessment assignment. Pretend that you
have discussed your client’s history with treatment, goals, and preferences for interventions prior to writing up this
document.
When writing up this treatment plan, follow APA (7th ed.) guidelines. Citations may not be needed to
complete this assignment, however if you refer to other sources such as the DSM-5 or articles, format the citations
and reference section correctly.
Your completed submission should be a 12-pt. font, double-spaced, WORD document with cover page which
includes all of the following elements. Keep the section headings but remove all of my descriptions. Each section
should be completed using complete sentences in paragraph formatting. Do not remove, skip, or neglect to respond
to any of the headings.
Re-“introduce” me to your client by beginning the paper with a case formulation. Then create the treatment
goals, intervention, and symptom tracking scheme. Demonstrate a connection between the theoretical framework
you are using to understand their presenting problem, diagnosis, and treatment recommendations.
Case Formulation – This is your assessment of the client – the aim is to synthesize the information you have just
gathered to help us understand why the client is experiencing these symptoms at this time. You can include
presenting problem and underlying problems, your impression of the client, any themes or patterns you were aware
of, strengths, weaknesses, cultural issues, motivation, readiness to change, what may help the client be successful,
negative factors that may impact treatment, preferred coping mechanism of client, potential support systems,
appropriate level of treatment, etc., and what psychological theory (e.g., psychodynamic, cognitive etc.) helps us to
understand the client’s symptom picture.
● Review the “Case Formulation Outline” document provided on Canvas for additional guidance on crafting
the case formulation.
Goals and Interventions –
● Long Term goal – Create 1 long term treatment goal which would be in line with the client’s presenting
problem and reason for seeking treatment at this time. Use specific language and note how you will know
when the goal is reached.
● Short Term goals – Create 3 short term SMART goals which aid in the client reaching the long-term goal.
Use specific language and note how you will know when the goal is reached.
● Intervention – Identify 1-2 interventions for each short-term goal that aligns with the client’s goals and
beliefs.
● Rationale — Present the connection between the theoretical framework you are using to understand the
presenting problem and symptoms, diagnosis, and treatment recommendations. Also, what would you need
to discuss with the client to determine if they are on board with this treatment plan?
Evaluation – Using the single-subject design, determine how you will track the frequency, rate, and/or intensity of
the symptom(s) and how you will determine if it is time to supplement or change the intervention.
Other – What other considerations need to be addressed for this client?
