construct a written proposal reflective of an evidence-based change initiative designed to address a problem/issue that is impacting a project site. Successful completion demonstrates achievement of Course Objectives 4, 5, 6, 7.
• Directions: Using the MAP-IT model described in Module 1, mobilize a project site team, conduct an organizational assessment, and plan a change initiative in collaboration with the site team. The written project proposal will follow the SQUIRE Guidelines. Refer to Module 4 and Assignments for additional information.
• Evaluation: Input/feedback from project site team, course peer, and Second Reader; grading completed by Project Faculty Advisor in accordance with associated rubric
o Students will revise the graded proposal based on faulty feedback and submit to a Second Reader for review. The second Reader will use the same rubric for evaluation and indicate, Exceeds, Meets, or Needs Improvement for each category
o Students and Project Faculty will establish a plan to resolve any areas of the proposal in need of improvement identified by the Second Reader
